Passports

Children Under 13 years of age

• Completed DSP-11 form - Both parents or legal guardians must sign form for child in the presence of Clerk's office staff and show their Drivers License or State Identification.
• Certified Birth Certificate form Health Dept. or a previous passport.
• 2 passport pictures (Please use true passport photo's)
• $40.00 cash (25.00 passport fee, $15.00 processing fee) or money order made payable to "Fountain County Clerk." NO PERSONAL CHECKS ACCEPTED.

Children 13-15 years of age

Adults 16 years of age and over

• Completed DSP-11 form - you need to be present in the clerks office to sign application in front of us. 
• We will need to see his/her drivers license or stable identification.
• Certified Birth certificate from the Health Dept. or a previous passport.
• 2 passport pictures (please use true passport photo's)
• $60.00 ($45.00 passport fee, $15.00 processing fee) cash or money order made payable to Fountain Co. Clerk. (NO PERSONAL CHECKS ACCEPTED)

Renewals

If you can answer YES to the following questions, you may submit your application for passport by mail.

• I can submit my most recent passport.
• I was at least 16 years old when my most recent passport was issued.
• I was issued my most recent passport less than 12 years ago.
• I use the same name as on my most recent passport, or I have had my name changed by marriage or court order and can submit proper documentation to reflect my name change.

If you answered No to any of the above questions - STOP - you are not eligible to use this form.

If you answered YES to the above questions you will need to do the following:

• Obtain Form DSP-82 "Application for Passport by mail" from the clerk's office.
• 2 passport pictures (please use true passport pictures)
• Previous passport issued to an adult within the last 12 years.
• $40.00 check or money order made payable to "Passport Services"

Applicant sends all the above to the address on the DSP-82 form.

Expedite

If you have travel plans that fall ahead of the normal processing time you may expedite your application.

Additional $35.00 fee per application

You will need two (2) overnight mailers and two (2) $12.25 stamps per application for express mailing. Bring these items to the Clerk's office when processing application.
